Hospice of Crossing Rivers Health is a philosophy of care whose mission is to enhance the quality of life for patients and their families faced with a terminal illness.
Our team of specialists
The hospice team works closely with patients and families to identify what their personal goals are and are dedicated to help achieve these.
The attending physician consults with the hospice team to plan care.
- Coordinate the plan of care.
- Teach and supervise home management of the patient.
- Provide nursing care as often as necessary, including emergency visits when needed.
The hospice medical director attends interdisciplinary team meetings and provides guidance to the team regarding the patient's plan of care.
- Assist with Advance Directives planning.
- Access community and financial resources.
- Provide counseling, support and guidance to the patient and family.
The chaplain provides spiritual support of all faiths for patients and families in coordination with the patient's own pastor, priest, or spiritual healer.
Home care aides assist with personal cares and occasional light housekeeping duties.
Alternative therapy volunteers provide relaxation techniques such as Reiki, massage, music and pet therapy.
Trained volunteers give support to hospice patients and their family members by offering a listening ear, a caring heart and a helping hand.
Grief support groups provide support and counseling for family members, along with periodic calls/mailings, to help cope with grief following loss.
Crossing Rivers Hospice provides support and peace at the end of life’s journey, creating a caring environment where family and friends can say goodbye. Specially trained registered nurses are available 24/7.
Hospice is provided in a variety of settings including private homes, assisted living facilities, nursing homes, and for short-term purposes, at area hospitals.
Bereavement support continues for families for up to 13 months after the passing of their loved one.

Crossing Rivers Health Hospice has been named a 2016 Hospice Honors recipient by Deyta Analytics, a division of HEALTHCAREfirst. Hospice Honors is a prestigious program that recognizes hospices providing the highest level of quality, as measured from the caregiver’s point of view.
Crossing Rivers Health Hospice program serves the southwest Wisconsin communities throughout Crawford County, along with portions of Grant, Vernon and Richland counties in Wisconsin, as well as portions of northeast Iowa, including Clayton and Allamakee counties.
