We invite you and your family to join us in reflection and
remembrance of your loved ones through a live virtual
Candlelight Memorial

Program includes words of comfort during this holiday season from our Hospice team and volunteers; names and photos of loved ones who have passed; and prayer.

Loved ones of Crossing Rivers Health Hospice patients, as well as family members of patients who have passed in the last year at Crossing Rivers Health, are invited to participate. Members of the community who wish to have their loved ones name included in the service may contact Haley Kregel, Crossing Rivers Health Hospice social worker, at 608-357-2449 or haley.kregel@crossingrivers.org. The deadline for submitting a name is November 19, 2021.
This year, family members are encouraged to submit a photo of their loved one to be displayed alongside their name and significant dates. Loved ones are to take note the photos they submit will not be returned. The photos can be submitted electronically to the email address listed above or mailed to Crossing Rivers Health attn: Hospice 37868 US Hwy 18 Prairie du Chien, WI 53821. Please be sure to include your loved ones name as you wish it to appear, date of birth, and date of death when submitting a photo.
As plans continue to be made, the Crossing Rivers Health Hospice and Bereavement team looks forward to connecting with you this holiday season though our annual Candlelight Memorial Service. The theme for the event this year is a “Country Patchwork Christmas”. Going along with the concept of the program, loved ones are encouraged to reflect and share precious memories and stories of those we have lost in the past year. As part of the virtual event on December 9th, there will be a prayer offered by Jerry Mays, hospice spiritual care volunteer, as well as poems, stories, and words of remembrance and appreciation by hospice and bereavement team members.
