Crossing Rivers Health Hospice is a philosophy of care whose mission is to enhance the quality of life for patients and their families faced with a terminal illness.

Hospice is provided in a variety of settings including private homes, assisted living facilities, nursing homes, and for short-term purposes, at area hospitals. Specially trained registered nurses are available 24/7.

Bereavement support continues for families for up to 13 months after the passing of their loved one.

Our Hospice team

The Hospice team works closely with patients and families to identify their personal goals and are dedicated to helping achieve them.

Attending physician

The attending physician consults with the hospice team to plan care.

Registered nurses

  • Coordinate the plan of care.
  • Teach and supervise home management of the patient.
  • Provide nursing care as often as necessary, including emergency visits when needed.

Hospice medical director

The hospice medical director attends interdisciplinary team meetings and provides guidance to the team regarding the patient's plan of care.

Social workers

  • Assist with Advance Directives planning.
  • Access community and financial resources.
  • Provide counseling, support and guidance to the patient and family.

Spiritual Caregiver

The spiritual caregiver provides spiritual support of all faiths for patients and families in coordination with the patient's own pastor, priest, or spiritual healer.

Hospice homecare aids

Hospice homecare aides assist with personal cares and occasional light housekeeping duties.

Alternative therapy volunteers

Alternative therapy volunteers provide relaxation techniques such as Reiki, massage, music and pet therapy.

Trained volunteers

Trained volunteers give support to hospice patients and their family members by offering a listening ear, a caring heart and a helping hand.

Grief support groups

Grief support groups provide support and counseling for family members, along with periodic calls/mailings, to help cope with grief following loss.

Service area

Crossing Rivers Health Hospice program serves the southwest Wisconsin communities throughout Crawford County, along with portions of Grant, Vernon, and Richland counties in Wisconsin, as well as portions of northeast Iowa, including Clayton and Allamakee counties.


Give to Hospice

Hospice care is provided at no cost to the patient and, in many cases, our patients’ needs are fulfilled without full reimbursement from Medicare, HMOs or insurances.
