March is Social Work Month

Prairie du Chien, WI - Crossing Rivers Health is joining the National Association of Social Workers this March in celebrating Social Work Month. National Professional Social Work Month is an opportunity for social workers across the country to turn the spotlight on the profession and highlight the important contributions they make to society.

Social workers confront some of the most challenging issues facing individuals, families, communities, and society and forge solutions that help people reach their full potential and make our communities a better place to live. At Crossing Rivers Health, social workers play a critical role in coordinating care for many of our patients throughout the entire organization.

Tara Nalepinski and Emily Wetter are social workers within the Crossing Rivers Health Patient Family Services team and play an active role in coordinating hospital discharges, locating appropriate financial and community resources, and providing emotional support for patients. Nalepinski and Wetter are trained to educate and assist patients, families, and the community – free of charge – about advance medical directives, such as power of attorney for health care and living wills.

“Tara and Emily coordinate care for patients once they are admitted to the hospital and also once they are discharged,” shared Amanda Schultz, Care Coordination Director at Crossing Rivers Health. “They are truly patient and family advocates and are invested in ensuring patients have everything they need for a successful hospital discharge.”

Brittney Miller and Haley Kregel are social workers within the Hospice department at Crossing Rivers Health. As Hospice social workers, Miller and Kregel serve hospice patients throughout portions of Crawford and Grant counties in Wisconsin, and Clayton and Allamakee counties in Iowa.

“The social worker role is so important to the development of a comprehensive hospice plan of care for the patient and family,” commented Laura Lane, Crossing Rivers Health Hospice and Palliative Care Manager. “Haley and Brittney do a wonderful of job of helping patients and families process very difficult information and go on to assist them in living their best final moments with family and friends.”
