Prairie du Chien, WI – Crossing Rivers Health Clinic announces the long-term care team of Matthew Olson, MD, and Michelle Mertens-Dodgen, MSN, APNP. The team is dedicated to caring for patients who reside in area assisted living and nursing homes and is currently accepting new patients. Dr. Olson and Michelle coordinate and provide comprehensive medical care in collaboration with the facility’s overall plan of care for their resident.
“We are excited to welcome Dr. Olson back to Crossing Rivers Health Clinic as a long-term care provider,” shared Maria Mathis, Crossing Rivers Health Clinic Director. “Michelle shares his passion for providing care to our older adults. Together, they are an amazing team and we are proud to be able to offer such a high level of compassionate care to residents living in area assisted living and nursing homes.”
As a team, Dr. Olson and Michelle evaluate and determine medical regimens for each individual resident. They provide scheduled visits appropriate to the resident’s medical condition, depending upon medical history and stability, and establish the regularity of visits based on the resident’s needs and requirements. Additional services include wellness exams, chronic disease management, medication management, progress updates for family and facility staff, and end-of-life planning.
