Swing Bed Program available at Crossing Rivers Health

Crossing Rivers Health is proud to offer a Swing Bed Program in our community. Swing Bed is for patients who need additional short-term rehabilitation care following an acute hospital stay. The goal of this program is to help patients return to their homes or with the support of other community resources if needed.

The Swing Bed program allows patients to transition, or “swing,” from acute hospital care to rehabilitation and skilled nursing care without leaving the hospital.

Amanda Schultz, Director of Care Coordination at Crossing Rivers Health says, “Not all patients are ready to go home after their hospital stay. We are extremely fortunate to have a Swing Bed Program so patients can continue their recovery close to home in our beautiful medical center supported by our skilled nursing staff and 24/7 in-house physician.”

Swing Bed care at Crossing Rivers Health is high-quality care in a beautiful environment, that is close to home. The private patient rooms are spacious, offer a gorgeous view, and have an in-room bathroom. Rooms are furnished with a sofa bed to accommodate visiting family members or friends during the patient’s extended stay.

A wide variety of services are offered, including skilled nursing services, physical and occupational therapy, speech and swallowing therapy, nutrition services, diabetes education, pharmacy support, respiratory therapy, discharge planning, personal care assistance, housekeeping/laundry, lab and medical imaging services, physician visits, and case management.

Most insurance companies require three overnight stays in an acute care hospital within the last thirty days; the care needed relates to the reason for the acute stay, and skilled nursing or skilled physical rehabilitative therapies are required daily.
