At Crossing Rivers Health, every aspect of your experience as a patient is approached with compassion, efficiency, and effectiveness. We strive to provide excellent care to our patients and their families.
Because of this commitment, Crossing Rivers Health was recognized with an Excellence in Patient Care Award for providing exemplary results related to the responsiveness by our staff as measured by the Hospital Consumer Assessment of Helathcare Providers and Systems (HCAHPS) survey.
Transforming Care at the Bedside
We are one of 16 Wisconsin hospitals participating in the Transforming Care at the Bedside (TCAB) grant program, funded by the Aligning Forces for Quality Initiative of the Robert Wood Johnson Foundation. The goal of the TCAB program is to encourage front-line staff to identify, plan and implement changes to improve efficiency, patient care and satisfaction, and staff vitality. Crossing Rivers Health has completed many improvement projects as a part of the TCAB program. These include quiet time, RN–physician rounding, stocked patient room closets, parking lots for equipment storage, RN–RN report sheet, recycling in patient rooms, discharge health care folders, and patient room service for meals, with more improvements to come in the future. Many of the projects are described in more detail below.
RN–physician rounding
Our registered nurses and physicians make rounds together to see patients. This improves communication among patients, nurses and medical providers.
We are better able to meet all patient needs. We have also reduced nonemergent phone calls to physicians, thereby reducing interruptions of patient clinic appointments.
Quiet time
Patients have an hour of undisturbed rest time from 1:00 to 2:00 p.m. every day. Patients are not scheduled for therapies, nursing interventions, tests or other activities, unless they are medically necessary. Follow-up surveys after quiet time was initiated have had very positive results and numerous positive comments from patients and staff.
Patient room service for meals
Nursing and nutrition services are developing patient room service for meals. Patients will be able to make their own food selections from menus specific to their dietary needs.
Hourly rounding
A registered nurse or certified nursing assistant visits each patient every hour during waking hours to see if the patient has any concerns. We make sure pain is under control, water and personal items are within reach, hygiene cares are completed, and more.
Recycling in patient rooms
Recycling receptacles for plastics, glass, metals and aluminum are available in patient rooms. This helps Crossing Rivers Health reduce the amount of garbage we produce.
