Community Health Needs Assessment
Crossing Rivers Health collaborates with several regional health care providers and agencies to conduct a Community Health Needs Assessment (CHNA).
The CHNA is conducted every three years to help identify needs which may exist in a variety of areas ranging from quality of life, education, socio-economic, resource or services, as well as health and wellness. Once the assessment is complete, needs are prioritized and strategies are then developed into a Community Health Implementation Plan to address those prioritized needs.
