Community Health Needs Assessment

Every three years, Crossing Rivers Health collaborates with several regional health care providers and agencies to conduct a Community Health Needs Assessment (CHNA).

The CHNA is conducted to help identify a needs, or gaps which exist, in a variety of areas ranging from quality of life, education, socio-economic, as well as health and wellness. Once the assessment is complete, needs are prioritized and strategies are then developed to address those prioritized needs.
