Diabetes is a serious disease and fortunately, many people live long and healthy lives by learning to manage their blood sugars through lifestyle changes and other self-management skills.

The Diabetes Self-Management Program provides comprehensive diabetes education for mastering self-care skills for your lifelong journey.

Our Diabetes Self-Management Program is available for people diagnosed with:

Type 1 Diabetes

  • Often occurs in teens or early adulthood.
  • 10% of people with diabetes have Type 1.

Type 2 Diabetes

  • The risk of Type 2 is increased in overweight, inactive & older people.
  • 90% of people with diabetes have Type 2.

Gestational Diabetes

  • Occurs during pregnancy.

Program benefits

There are many benefits of working with a diabetes educator at Crossing Rivers Health. You will:

  • Develop a diabetes management plan that matches your lifestyle, behaviors, and medications.
  • Learn about healthy eating tips so you don’t have to give up all the foods you like or eat different meals than your family.
  • Learn how to take accurate blood sugar readings.
  • Have ongoing appointments to make sure your diabetes is under control and to adjust your treatment plan, as needed.
  • Be part of an education program that follows national standards set by the American Diabetes Association.

Participating in the diabetes program is very important and can help you manage your health. Other important diabetic levels to monitor:

DAILY

  • Check blood sugars, as directed by your Primary Care Provider

EACH CLINIC VISIT

  • Blood pressure check

EVERY 3-6 MONTHS

  • Hemoglobin A1c lab check

AT LEAST ONCE A YEAR

  • Cholesterol
  • Kidney test
  • Eye exam
  • Foot exam
  • Dental exam
  • Flu vaccine
