You can rely upon our compassionate team of caregivers to ensure your needs, wishes, and goals are met while supporting your loved ones along the way. The Hospice team works closely with patients and families to identify their personal goals and are dedicated to helping achieve them.
Attending Physician
The Attending Physician consults with the hospice team to plan care.
Registered Nurses
Registered Nurses coordinate the plan of care; teach and supervise home management of the patient; and provide nursing care as often as necessary, including emergency visits when needed. Meet your Hospice Registered Nurses.
Hospice Medical Director
The Hospice Medical Director attends interdisciplinary team meetings and provides guidance to the team regarding the patient’s plan of care.
Social Workers
Social Workers assist with Advance Directives planning; access community and financial resources; provide emotional support and guidance; coordinate spiritual support from their own pastor, priest, or spiritual healer in the community to help the patient and family. Certified Nursing Assistants
Certified Nursing Assistants assist with personal cares and occasional light housekeeping duties.
Hospice Volunteers
Trained Volunteers give support to hospice patients and their family members by offering a listening ear, a caring heart and a helping hand Grief Support Groups provide support and counseling for family members, along with periodic calls/mailings, to help cope with grief following a loss.
Alternative therapy volunteers provide relaxation techniques such as Reiki, Music, and Pet Therapy.
