Empower Your Future: A Community Program on Advance Care Planning

Hospital news | Tuesday, April 2, 2024

Can you imagine being too sick to make your own healthcare decisions? Or suffering a critical injury and not being able to act on your own behalf? The decisions about end-of-life care are deeply personal, and sometimes very difficult to discuss with your family.

On April 16, in recognition of National Health Care Decision Day, Crossing Rivers Health Advance Care Planners are hosting free community programs that will focus on the importance of advance care planning and having a conversation with family and health providers, about how to start the discussion, and how to document your decisions before it’s too late. The sessions, scheduled for 30 minutes each, will take place at 12:00 p.m., 4:00 p.m., and 6:00 p.m. in the George Family Education Center at Crossing Rivers Health.

Tara Nalepinski, Social Worker at Crossing Rivers Health said, “Advance Care Planning gives you the power to shape your medical care, including the freedom to accept or decline treatments. Planning for the future means expressing your wishes now, ensuring they're honored even if you can't speak for yourself later on.”
