Community Health Needs Assessment & Improvement Plan
Crossing Rivers Health collaborates with several regional healthcare providers and agencies to conduct a Community Health Needs Assessment (CHNA).
The CHNA is conducted every three years to help identify needs in various areas, such as quality of life, education, socio-economics, resources or services, and overall community health and wellness. Once the assessment is complete, needs are prioritized and strategies are then developed into a Community Health Implementation Plan (CHIP) to address those prioritized needs.
