What does the referral process look like?
- Schedule an appointment with your Crossing Rivers Health Primary Care provider to request a referral to one of our integrated behavioral health team members or attend a regularly scheduled primary care visit.
- Mention to your provider that you would like to meet with a behavioral health counselor or your provider may identify a behavioral health need during your visit and recommend a referral.A behavioral health provider will come to your exam room to meet with you on the same day of your appointment, if possible.
- You may be scheduled for a few more follow-up visits (4-6 total visits) with your behavioral health provider to address your short-term behavioral health needs.
- For longer-term or more involved behavioral health needs, your primary care or behavioral health provider may refer you to the Center for Behavioral Health for outpatient care. There, a team of therapists and psychiatric providers is ready to help you address all of your behavioral health concerns.
